Selecting a Date
Timing is Everything
We understand the importance of time management. Smile Premier wants to ensure that the date selected works best for the school and the students. Upon date selection, we coordinate with the teachers to make certain that their educational timeline is not disrupted.
Consent Form Distribution
Parent Signature
Consent forms are distributed by the teachers to each student to be completed by parents, and then returned to administration. Consent forms that are missing information will be returned to parents who still want their child to be seen.
